All living persons residing in the United States whose Private Information was potentially or actually compromised in the Mt. Baker Imaging and Northwest Radiologists data security incident that occurred between January 20-25, 2025. Excluded are directors or officers of the defendants, the assigned judge, the judge's immediate family, and court staff.
How to file
Submit a Valid Claim Form postmarked or submitted online by August 19, 2026. The form can be submitted at the settlement website. You may receive reimbursement for documented Out-of-Pocket Losses (up to $5,000), a Pro Rata Cash Fund Payment, and two years of medical identity-theft protection.
