Do you qualify?
Anyone who paid any amount of money for a metformin-containing drug intended for personal or household use that was manufactured, distributed, or sold by Teva, Granules, or Heritage between July 20, 2015 and June 2, 2020. Third-party payors who paid or reimbursed for such drugs also qualify.
