$3,500 eachYou may submit a Claim Form and provide reasonable documentation for losses related to fraud and/or identity theft as a result of the Data Incident. Up to $3,500 per Settlement Class Member can be claimed.
Medical Data Monitoring
In addition to the Cash Payment for Documented Losses, you may also elect to receive three years of free Medical Data Monitoring.
