EyeMed failed to use appropriate data security, which led to an unauthorized third-party accessing patient personal data including names, contact details, dates of birth, health insurance information, Medicare/Medicaid numbers, driver's licenses, and government identification numbers in a data incident that occurred in June 2020. How to file

You can claim a $50 pro rata cash payment, up to $100 for lost time, and/or up to $10,000 for documented out-of-pocket losses. Documentation is required for out-of-pocket loss claims.
