Lucent Health Solutions Data Breach Settlement

What happened

Lucent Health Solutions, LLC suffered a targeted cyberattack in October 2023 that compromised certain computer systems. Files containing private information such as names, dates of birth, health/dental/vision policy numbers and member IDs, and plan numbers may have been accessed. The defendant denies wrongdoing.

Do you qualify?

Anyone whose private information was compromised in the October 2023 LH Data Incident, including personal information such as full name, date of birth, and health, dental, or vision policy or plan information.

What you can claim

• Reimbursement for Documented Ordinary Out-of-Pocket Expenses

  $550 each

  Class members who incurred actual, documented out-of-pocket expenses due to the Data Incident between October 2, 2023, and September 5, 2026, can be reimbursed up to $550.00. This covers expenses such as credit report fees, ID replacement costs, and postage for contacting banks.

• Reimbursement for Documented Extraordinary Out-of-Pocket Expenses

  $5,500 each

  Class members who lost money due to identity theft or fraud probably caused by the Data Incident can be reimbursed up to $5,500.00, provided losses occurred between October 2, 2023, and September 5, 2026, and are not already covered by other benefits or third-party reimbursement.

• Reimbursement for Lost Time

  $125 each

  Class members who spent time responding to the Data Incident may claim up to five hours at $25.00 per hour for a maximum of $125.00 for tasks such as changing passwords, investigating suspicious activity, or researching the Data Incident.

• Alternative Cash Payment

  $80 each

  Instead of other cash payment options, class members may claim a one-time $80.00 payment with no proof or explanation required.

Who is in the class

All whose Private Information was implicated in the Data Incident, including all individuals to whom Defendant sent an individual notification letter to regarding the Data Incident.

How to file

Submit a valid Claim Form online at the settlement website, by downloading and mailing it to the Settlement Administrator, or by calling or emailing the Settlement Administrator to request a paper copy. The deadline to submit a claim is September 5, 2026.
