ProSmile Holdings suffered a targeted cyberattack on or about July 7, 2022, in which certain files containing private information were accessed. Do you qualify?
Anyone who received notice from the Settlement Administrator indicating they are a Class Member. This includes individuals whose personal information was potentially accessed in ProSmile's data security incident on or about July 7, 2022.
How to file
File a Claim Form online at the settlement website or download, print, and mail it to the Settlement Administrator by September 25, 2025. Include documentation of any out-of-pocket losses (up to $5,000). You can choose to receive payment digitally via PayPal, Venmo, or Zelle, or select either a Pro Rata Cash Payment or Social Security Number Impact Payment.
