What happened

Netgain Technology, LLC, an IT and cloud computing service provider for healthcare and accounting companies, experienced a data breach between September 2020 and November 2020. An unauthorized individual accessed Netgain's computer systems and disclosed personally identifiable information and private health information that Netgain stored on behalf of its business clients.

Anyone who received a notice from a healthcare provider or accounting company between September 2020 and November 2020 referencing the Netgain data breach and resides in the United States. Specifically, individuals whose personal or health-related information was potentially disclosed to an unauthorized third party during the breach.

• Documented out-of-pocket expense reimbursement

  $5,000 each

  Reimbursement of up to $5,000 for documented out-of-pocket expenses related to the data breach, including fraud-related losses, credit monitoring costs, and other unreimbursed charges, plus up to $75 for attested time spent (3 hours at $25/hour).

All individuals who reside in the United States and who potentially had their personal or health-related information disclosed to an unauthorized third party between September 2020 and November 2020 in the course of a databreach experienced by Netgain.

How to file

Submit a valid Claim Form by September 17, 2025, either via U.S. mail or online. For documented out-of-pocket expenses, you must provide supporting documentation in the form of an attestation to the amount of time spent and how the time was spent (capped at 3 hours at $25/hour). For other eligible expenses like fraud-related losses or credit monitoring costs, submit supporting documentation of the charges.
