Do you qualify?

You qualify if you were a State Farm insured in Washington whose vehicle had repair estimates of at least $1,000 under UMPD coverage, the vehicle was no more than six years old with less than 90,000 miles at the time of accident, the vehicle suffered structural or body damage, and the accident occurred between March 23, 2012 and February 13, 2024. Excluded: leased vehicles, total loss vehicles, and accidents outside the specified date range.

Submit the enclosed blue Claim Form, sign it, and file it online or by mail postmarked by November 12, 2024. If you had multiple qualifying accidents, submit a separate Claim Form for each. The deadline has passed as of the notice date.
