Do you qualify?
All individuals residing in the United States whose PII or PHI was identified as at issue in the data incident discovered by AACOM in September 2024, including all those who received notice of the data incident.
How to file
Submit a Claim Form online or mail a completed paper claim form postmarked no later than March 4, 2026. You must provide reasonable supporting documentation for ordinary losses caused by the data incident to claim up to $3,500 in out-of-pocket compensation, or you may claim an estimated $50 cash payment without documentation.
