Anyone who was previously notified that their personal information may have been compromised in the data security incident at Ciox Health, LLC d/b/a Datavant Group or whose personal information may have been affected by the incident.
How to file
Submit a valid Claim Form online or mail a completed paper Claim Form postmarked no later than August 18, 2026. This is the only way to receive benefits from the settlement.
