On May 1, 2023, Deanco Healthcare experienced a cybersecurity attack that compromised its IT network. How to file

Submit a Claim Form online or by mail by August 12, 2026. To receive a Documented Loss Payment (up to $5,000), you must provide proof of your losses. A California Statutory Payment of $100 is available to eligible Class Members. Class Members can also elect to receive two years of Medical Monitoring Services and/or a pro rata share of residual funds.
