What happened
On May 1, 2023, Deanco Healthcare experienced a cybersecurity attack that compromised its IT network. The threat actor accessed database files containing customers' addresses, dates of birth, Social Security numbers, driver's license numbers, financial account information, health insurance details, and clinical information. Deanco agreed to settle without admitting wrongdoing.
Do you qualify?
Anyone who was notified on or around November 21, 2023, by Deanco or a Deanco affiliate that their Personally Identifying Information may have been compromised in the May 1, 2023 data security incident.
How to file
Submit a Claim Form online or by mail by August 12, 2026. To receive a Documented Loss Payment (up to $5,000), you must provide proof of your losses. A California Statutory Payment of $100 is available to eligible Class Members. Class Members can also elect to receive two years of Medical Monitoring Services and/or a pro rata share of residual funds.
