On May 1, 2023, Deanco Healthcare experienced a cybersecurity attack that compromised its IT network. A threat actor accessed database files containing sensitive personal information including addresses, dates of birth, Social Security numbers, driver's license numbers, financial account information, health insurance plan member IDs, claims data, and clinical information. Deanco has agreed to settle the resulting class action litigation.
Do you qualify?
Anyone who was notified on or around November 21, 2023, by Deanco Healthcare or a Deanco affiliate that their Personally Identifying Information may have been compromised because of the May 1, 2023 Data Security Incident.
How to file
Submit a Claim Form online or by mail postmarked by August 12, 2026. To receive a Documented Loss Payment (up to $5,000), you must provide documentation of incurred losses. All eligible Class Members can receive the $100 California Statutory Payment and Medical Monitoring Services for two years without proof of loss.
