What happened
Frontier Communications Parent, Inc. suffered a data breach detected on or about April 14, 2024, in which an unauthorized third party may have gained access to files containing private information (names, dates of birth, Social Security numbers, and other personally identifiable information) of individuals who applied to Frontier for residential services. The company detected the breach and notified affected individuals in or around June 2024.

Any living individual residing in the United States who received notice of the data breach from Frontier in or around June 2024 indicating that their private information may have been impacted in the April 14, 2024 data incident.
How to file
Submit a timely Claim Form either online or by mail (postmarked) by October 27, 2025. You can either claim a flat $100 payment without documentation, or submit documentation of losses up to $5,000. You may also claim two years of free credit monitoring in addition to a cash payment.
