Ordinary Losses Reimbursement
$500 eachReimbursement for documented out-of-pocket expenses incurred between June 20, 2024, and July 23, 2026, as a result of the Data Security Incident, including unreimbursed bank fees, phone charges, data charges, postage, gasoline for local travel, and fees for credit reports or identity theft insurance products. Maximum $500.00 per class member.
Extraordinary Losses Reimbursement
$5,000 eachReimbursement for documented monetary losses caused by misuse or fraud of Private Information that occurred between June 20, 2024, and July 23, 2026, provided the loss was more likely than not caused by the Data Security Incident and the class member made reasonable efforts to seek reimbursement through other means. Maximum $5,000.00 per class member.
Lost Time Reimbursement
Reimbursement for attested lost time reasonably related to mitigating the effects of the Data Security Incident at $25.00 per hour, up to 4 hours maximum per class member.
Information Security Measures
Defendant has implemented and will continue to maintain appropriate security measures to mitigate risks to information security.
