Hafetz denies wrongdoing but agreed to settle the class action lawsuit.

How to file
Submit a claim form by January 22, 2025 (postmarked or submitted by that date). For unreimbursed economic losses up to $10,000, you must provide supporting documentation. You can also receive two years of credit monitoring services and a pro-rata cash payment of $50 (adjusted based on remaining funds).
