Do you qualify?
All living persons residing in the United States whose Private Information was potentially or actually compromised in the Mt. Baker Imaging and Northwest Radiologists Data Security Incident between January 20 and January 25, 2025. Excluded are directors and officers of the Defendants, the assigned Judge, and Court staff.
How to file
Submit a Valid Claim Form online or by mail postmarked by August 19, 2026. To receive reimbursement for Out-of-Pocket Losses, you must provide documentation of those losses (up to $5,000). All claims must be submitted by the deadline to be eligible for any Settlement Benefits.
