$2,500 eachReimbursement for actual, documented out-of-pocket losses due to the Data Incident that occurred between June 1, 2025, and July 8, 2026, including identity theft/fraud losses, credit report/monitoring fees, ID replacement costs, and postage. Requires proof such as bank statements or receipts.
Alternate Cash Payment
$75 eachA one-time flat cash payment available as an alternative to documented loss reimbursement. No proof or explanation is required to claim this payment.
