What happened
PRC-Saltillo experienced a targeted cyberattack on its computer systems in August 2024 in which files containing sensitive information were accessed. The accessed information may have included names, addresses, phone numbers, dates of birth, treatment cost information, physician information, health insurance policy numbers, Medicare/Medicaid plan names, medical devices purchased, and certain employee Social Security numbers. PRC denies any wrongdoing.

How to file
Submit a Claim Form by December 31, 2025. You can submit online through the settlement website, download and mail the form to the Settlement Administrator, or call/email the Settlement Administrator to receive a paper copy or if you lost your Notice ID.
