$5,000 eachSettlement Class Members may submit a claim for reimbursement of up to $5,000.00 for documented out-of-pocket expenses incurred as a result of the Data Incident, including unreimbursed costs for addressing identity theft, fraud, or misuse of Personal Information. Requires documentation supporting the claimed losses.
Cash Payment
$75 eachIn the alternative to reimbursement for documented losses, Settlement Class Members who submit a timely Valid Claim may elect a Cash Payment. The exact amount depends on the total remaining funds after payment of other claim types.
