What happened
Between September 27 and October 9, 2023, an unauthorized third party gained access to personal information of Azura Vascular Care patients. The compromised data could include names, addresses, dates of birth, Social Security numbers, driver's license numbers, medical records, and other sensitive patient and billing information. Azura agreed to pay $3.15 million to settle claims arising from this data breach.

All natural persons whose personal information may have been compromised in the data breach between September 27, 2023, and October 9, 2023, including all persons who were sent notice of the data breach by Azura.
How to file
Complete and file a Claim Form online or by mail postmarked by June 30, 2025. For documented loss payments up to $10,000, you must submit supporting documentation such as credit card statements, bank statements, invoices, or receipts. Alternatively, you can file a claim with no documentation to receive a pro rata cash payment from the settlement fund.
