On or about March 24, 2023, unauthorized third-party access to NCB's systems resulted in the compromise, theft, or unauthorized access to Settlement Class Members' personal information.

Submit a Settlement Claim Form by August 26, 2025 via the Settlement Website at https://ncbdatasettlement.com/ or mail it to Kroll Settlement Administration LLC, P.O. Box 225391, New York, NY 10150-5391, postmarked on or before August 26, 2025. You must submit your own claim form to receive any settlement benefits.
