You are included in the Settlement Class if you were sent a notice of the Data Breach on or around December 18, 2023 informing you that your personal information may have been compromised.
How to file
Submit a Claim Form online by September 14, 2026 or mail your completed paper Claim Form postmarked no later than September 14, 2026. You can claim reimbursement for out-of-pocket losses and lost time, or receive an Alternative Cash Payment. All Settlement Class Members automatically receive an enrollment code for Identity Defense Services and Restoration Services.
