Anyone who used Southern Illinois Healthcare Enterprises, Inc., Southern Illinois Hospital Services, or Southern Illinois Medical Services and either logged into or scheduled an appointment through MyChart Patient Portal between February 21, 2022 and February 28, 2023, or June 12, 2023 and July 6, 2024; or completed a Health Risk Assessment between November 2, 2020 and January 31, 2021, or February 1, 2021 and April 30, 2021.
