How to file
Complete and submit a Claim Form by July 21, 2026. You can submit online using the Claimant ID and Password from your notice, or download a paper form and mail it so it is postmarked by the deadline. You may choose reimbursement for ordinary losses (up to $2,500) and extraordinary losses (up to $10,000), or take a $100 alternative cash payment. All claimants can also receive two years of identity theft protection services.
