Do you qualify?
All living individuals residing in the United States who were sent a notice of the data incident indicating that their private information may have been impacted. Excluded are company directors, officers, agents, governmental entities, the assigned judge and court staff, and those who opt out.
How to file
Submit a claim form by mail or postmark on or before Monday, March 16, 2026. You can claim up to $5,000 with documentation of losses related to the data incident, or receive an alternative $50 payment with no proof required. You may also elect medical data monitoring for two years by calling (833) 630-8366.
